Gynecomastia — excess breast tissue in men — affects an estimated 40–60% of males at some point in their lives. It's one of the most psychologically impactful cosmetic conditions and, despite its prevalence, one of the least talked about. Most men research this procedure privately, and the cost barrier keeps many from pursuing it.
In the US, gynecomastia surgery costs $5,000–$10,000 and is almost never covered by insurance (classified as cosmetic). In Colombia, the same procedure — using the same VASER or PAL liposuction technology — costs $2,000–$3,500. This is an outpatient procedure with a 5–7 day trip and minimal recovery.
Cost Comparison
| Technique | US Cost | Colombia Cost |
|---|---|---|
| Liposuction only (fatty gynecomastia) | $4,000–$7,000 | $1,500–$2,500 |
| Excision only (glandular tissue) | $5,000–$8,000 | $2,000–$3,000 |
| Combined lipo + excision (most common) | $5,000–$10,000 | $2,000–$3,500 |
| Severe gynecomastia (with skin excision) | $8,000–$12,000 | $3,000–$4,500 |
Grades of Gynecomastia
| Grade | Description | Typical Treatment |
|---|---|---|
| Grade I | Small enlargement, no skin excess | Liposuction alone or observation |
| Grade II | Moderate enlargement, no skin excess | Liposuction + glandular excision |
| Grade III | Moderate enlargement with mild skin excess | Liposuction + excision, possible minor skin tightening |
| Grade IV | Marked enlargement with significant skin excess | Excision with skin reduction (periareolar or full excision pattern) |
The Procedure
Gynecomastia surgery is typically outpatient, performed under general anesthesia or IV sedation. The procedure takes 1–2.5 hours depending on severity.
- Liposuction: A small cannula removes excess fatty tissue through 2–3 tiny incisions (3–5mm). VASER or PAL-assisted liposuction offers more precise fat removal and skin tightening compared to traditional suction.
- Glandular excision: A small incision along the lower edge of the areola removes firm glandular tissue that liposuction alone cannot address. This scar heals nearly invisibly along the areolar border.
- Skin excision: For Grade IV gynecomastia with significant skin excess, additional skin removal may be necessary — either a periareolar approach (scar around the areola) or, in severe cases, a pattern similar to female breast reduction.
Recovery
- Day 1: Compression vest applied. Mild soreness and swelling. Most patients walk and move freely.
- Days 2–3: Swelling increases then plateaus. You'll wear the compression vest 24/7 for 3–6 weeks.
- Days 5–7: Cleared to fly home. Light activity only.
- Week 2: Return to desk work. Driving comfortable.
- Week 4–6: Return to gym (upper body exercises). Compression vest transitions to daytime only.
- Month 3–6: Final results visible as swelling fully resolves. Chest contour defined.
Trip Planning
- Trip length: 5–7 days
- Hospital stay: Outpatient (go home same day)
- Companion: Not required but recommended for Day 1
- Concealment: The compression vest is worn under clothing and is not visible. Return to social activities within 3–5 days.
