Rotator cuff repair: procedure, tendon healing, and rehabilitation
Rotator cuff repair reattaches torn shoulder tendon to bone. Many tears are treated nonsurgically first; surgery is considered when pain, weakness, tear characteristics, or activity demands make repair appropriate.
Rotator cuff repair is fundamentally a tendon-to-bone healing operation, which is why the repair needs protection long after the skin incision looks healed.
When surgery may be considered
AAOS notes that continued pain despite nonsurgical treatment is a common reason for surgery. Other factors can include symptoms lasting months, large repairable tears, substantial weakness, high activity demands, or a recent acute tear.
Nonsurgical treatment first
Physical therapy, activity modification, anti-inflammatory treatment, and selected injections can improve symptoms in some patients. A structural tear can remain present even when symptoms improve.
Arthroscopic repair
Most modern repairs are performed arthroscopically through small portals. A camera visualizes the shoulder while anchors and sutures reattach tendon to the humeral head.
Open and mini-open repair
Open approaches are still used in selected situations, including complex tears or associated reconstruction. The label matters less than why the surgeon needs that exposure.
What else may be done
Depending on findings, the surgeon may address the biceps tendon, remove inflamed tissue, treat the acromion, or manage other shoulder pathology. Ask which additional procedures are planned.
The sling phase
The tendon needs protection. Many protocols use a sling for a period after surgery, but duration and when passive or active motion begins depend on tear size, tissue quality, fixation, and surgeon preference.

Tendon healing is slow
Feeling better does not mean the tendon-bone interface has fully matured. Patients may remain restricted from lifting or forceful active use even after pain improves.

Tear size and tissue quality matter
A small acute tear in healthy tendon is not the same problem as a massive chronic tear with muscle atrophy and fatty degeneration. MRI findings, tendon mobility, muscle quality, and chronicity influence whether a tear is repairable and how durable repair may be.
What if the tear cannot be repaired?
Depending on age, anatomy, arthritis, and function, alternatives can include debridement, partial repair, biceps procedures, tendon transfer, superior capsular reconstruction, or reverse shoulder replacement. These are different operations with different goals.
Sleep and daily-life planning
Early recovery can be difficult because dressing, bathing, sleeping, cooking, driving, and putting on a seatbelt all use the operative arm. Ask whether you need help at home and how to position the shoulder for sleep during the protection phase.
